Partner Program
There are five regional Primary Care Advisory Boards being established across the state as part of Georgia Memory Net.
While these Advisory Boards are established to facilitate care transitions between local PCPs and their nearest Georgia Memory Net Memory Assessment Clinics, these Primary Care Advisory Boards will provide us with a connection to these locations and nearby rural locations for partnership and practice improvement.
