Partner Program
The University of Georgia team works in collaboration with the Georgia Division of Aging Services to provide evidence-based services for seniors in Northeast Georgia. The team is led by Dr. Lesley Clack along with Graduate Research Assistant, Kyle Hunsinger.
For Georgia Gear, the University of Georgia team focuses on creation and maintenance of the Georgia HealthMatters website, which is used to facilitate patient referrals to evidence-based programs for seniors in Georgia. The website houses a HIPAA-compliant provider portal which was developed to allow healthcare providers to submit referrals securely for seniors who would benefit from services.
